Transition Services for High School Students with Disabilities
TEAM envisions that high school students with disabilities will become independent adults working and living in the community. The goal is to improve long-term outcomes by enabling students to engage in productive and meaningful daytime activities, by enrolling in post-secondary education or a vocational training program, or by securing employment within six months of graduation. Transition Education Advocacy and Mentoring Program provides advocacy to assist high school students transition to the adult world. The advocate informs students about options and resources and provides intensive follow up to ensure students are linked with, and receive needed services.
